1. Check to see if your employer matches funds. If yes, follow your employers directions before proceeding to step 2. If no, proceed to step 2.
2. Visit the UofI Fund's EFT page
3. Click "EFT Authorization Form"
4. Fill out the Authorization form. In the "I designate that my gift be used by" field, enter "330703 - Illinois Men's Lacrosse Club".
5. Mail the form to:

Urbana, IL 61801
6. A reciept for tax-deductible purposes will be sent to you by the Foundation.
Method 2: One Time Online Donation
1. Check to see if your employer matches funds. If yes, follow your employers directions before proceeding to step 2. If no, proceed to step 2.
2. Visit the UofI Fund's online donations page
3. In one of the "other" fields, type "330703 - Illinois Men's Lacrosse Club" and enter your desired donations amount.
4. Enter your payment information and click "Process Gift"
5. A reciept for tax-deductible purposes will be sent to you by the Foundation.
Method 3: One-time Donation by Mail
2. If you would like to donating using a credit card or check by mail, please follow these instructions.
Make checks or money orders payable to the "University of Illinois Foundation". Be sure to include a note indicating that the donation is to be made to "fund 330703 - Illinois Men's Lacrosse Club".
4. A reciept for tax-deductible purposes will be sent to you by the Foundation.
